Hospice Care

Hospice is specialized care that addresses the unique needs of a patient and family coping with the many issues that arise during the final stages of life.

Criteria for Hospice Home Care under Medicare.

  • Intermittent visits by nursing personnel, a homemaker, home health aide, social worker, chaplain, volunteers and other professionals as defined on the Informed Consent Form.
  • Intermittent visits and/or consultation by auxiliary team members providing dietary, physical occupational and speech therapies.
  • Respite care to relieve the family/caregiver for up to five (5) days per stay per period.
 

Home Health Care

Criteria for Home Health Care under Medicare:

  • Patient must be Home-bound and unable to function outside of the home without a considerable and taxing effort (i.e. with the aid of another person and/or device).
  • Have an exacerbation of an existing illness of onset of a new disease process
  • SN, PT, or SLP need on an intermittent basis
  • Administer IM injections
  • Teach administration when appropriate
  • Supervise administration
